Is IUI the Right Starting Treatment? Understanding Candidate Selection

Key Takeaways

  • IUI may be considered when at least one fallopian tube is open, ovulation is possible, and suitable sperm are available.
  • Age, sperm quality, infertility duration and previous treatment help decide whether IUI is a sensible starting option.
  • IUI is less involved than IVF, but it still needs monitoring and a clear review plan.
  • You do not have to try IUI before IVF if your medical findings suggest otherwise.

IUI treatment eligibility is not decided by one scan or a semen report alone. If you are wondering whether to start with IUI, your doctor needs to look at both partners’ findings, your age and how long you have been trying.

Is IUI the Right Starting Treatment? Understanding Candidate Selection

Many couples ask, “Can we try the simpler treatment first?” That is a reasonable question. But simpler treatment is useful only when it has a reasonable medical basis.

Intrauterine insemination, or IUI, places prepared sperm inside the uterus around ovulation. Fertilisation still needs to happen inside the body, usually in a fallopian tube. IUI does not bypass blocked tubes or correct every sperm problem.

What Determines IUI Treatment Eligibility?

Your doctor will decide whether IUI fits your situation after reviewing the couple together. One reassuring result cannot cancel out another finding that makes pregnancy through IUI unlikely.

At least one fallopian tube should be open

An egg and sperm need a way to meet. Doctors commonly assess whether the tubes are open using an HSG or another suitable test. An open tube is reassuring, although it does not prove that the tube functions normally.

If both tubes are blocked, IUI generally cannot help. With one open tube, the location of the developing follicle and other findings may affect the cycle plan.

Ovulation must occur or be induced

You may ovulate regularly on your own, or you may need medicines to help an egg develop. Ultrasound monitoring helps the doctor assess follicle growth and plan insemination.

For someone with PCOS, irregular ovulation does not automatically mean IUI is needed. Depending on the other findings, ovulation treatment with timed intercourse may be considered first.

Enough moving sperm should be available

The semen assessment looks at sperm count, movement and other characteristics. For IUI, the number of moving sperm available after laboratory preparation also matters.

There is no single cut-off that suits every couple or laboratory. Markedly reduced sperm numbers or movement may make IVF with ICSI more appropriate. An abnormal semen result may need repeat testing.

Who May Be Offered IUI?

IUI treatment eligibility often comes up when the basic evaluation does not show a major barrier to fertilisation. Situations where a doctor may consider it include:

  • Unexplained infertility: Initial tests have not identified a clear cause, but pregnancy has not happened.
  • Mild male-factor infertility: Enough moving sperm remain available after preparation.
  • Difficulty with intercourse or ejaculation: Insemination may help address a practical barrier to conception.
  • Use of donor sperm: IUI may be an option after appropriate screening, counselling and consent.
  • Selected cases of mild endometriosis: Age, symptoms, tube status and other findings guide the decision.

These are possibilities, not automatic approvals. For example, two couples with unexplained infertility may receive different advice because their ages and time spent trying are different.

Which Tests Help Make the Decision?

A useful assessment answers specific questions rather than adding tests without explanation. Bring earlier reports, including semen analyses, scans, prescriptions and records of previous treatment.

  • Medical and menstrual history: Cycle patterns, previous pregnancies, pelvic infections, operations and ongoing health conditions.
  • Semen analysis: To assess sperm availability and identify findings needing further review.
  • Pelvic ultrasound: To assess the ovaries, uterus and developing follicles.
  • Tubal assessment: To check whether at least one fallopian tube is open.
  • Selected blood tests: Hormone tests when indicated, along with infection screening and pre-pregnancy checks.

AMH and antral follicle count can help estimate ovarian reserve and likely response to medicines. They do not directly measure egg quality or, by themselves, determine whether you can conceive through IUI.

When Another Approach May Make More Sense

Choosing IVF directly is not “giving up” on a simpler option. Sometimes it avoids spending time on treatment that cannot address the underlying problem.

FindingWhat it may mean for treatment
Both fallopian tubes blockedIVF is usually considered because it bypasses the tubes.
Severely reduced usable spermIVF with ICSI may be discussed after further assessment.
Advanced endometriosis or significant pelvic diseaseIUI may be less suitable; treatment needs an individual plan.
Increasing age with limited time for treatmentThe doctor may recommend an earlier discussion about IVF.
Several unsuccessful, appropriately timed IUI cyclesA review may be more useful than repeating the same plan.

You can read about IVF treatment in Lajpat Nagar if this option is being discussed. Not everyone having IVF needs ICSI; your doctor should explain the reason for recommending either approach.

Agree on a Plan Before the First Cycle

Meeting IUI treatment eligibility criteria does not guarantee pregnancy. Before starting, ask what makes IUI reasonable for you and when the recommendation would change.

  • Will this be a natural cycle or a cycle using fertility medicines?
  • How will follicle growth and ovulation be monitored?
  • What findings would lead to postponing or cancelling insemination?
  • After how many attempts will we review the plan?

Medicines can lead to more than one follicle developing, increasing the chance of a multiple pregnancy. Ovarian hyperstimulation can also occur, particularly with injectable medicines. Monitoring helps manage these risks but does not remove them completely.

If too many follicles develop, your doctor may cancel the cycle and advise avoiding unprotected intercourse. Ask for clear instructions rather than assuming a cancelled insemination means there is no pregnancy risk.

Planning Visits Around Delhi NCR Life

IUI timing follows ovulation, not your office calendar. Ask about likely monitoring visits and sample collection instructions before arranging leave. The IUI treatment page can help you prepare questions for your consultation.

Pravi Global IVF is at E-3, 3rd Floor, Lajpat Nagar 2, New Delhi. Couples travelling from Noida, Ghaziabad, Gurugram or Faridabad may use the Metro with interchanges, or come by road depending on traffic.

Lajpat Nagar Metro is on the Violet and Pink lines. From the station, an auto or e-rickshaw may help with the last stretch. Market traffic and parking can need extra planning, especially for a scheduled sample appointment.

Frequently Asked Questions

Does age alone decide whether I can have IUI?

No. Age affects egg quality and how much time it makes sense to spend on treatment. Your doctor considers it alongside tube status, sperm findings, ovarian reserve and infertility duration.

Can I have IUI if one tube is blocked?

Possibly, if the other tube is open and suitable. The cause of the blockage, the side of follicle development and other findings need review before proceeding.

Does low AMH mean I am not eligible?

Not automatically. Low AMH may suggest fewer available follicles and influence treatment planning. Age and the overall assessment matter more than using AMH as a pass-or-fail test.

How many IUI cycles should we try?

There is no fixed number for everyone. Agree on a limited trial and review point with your doctor, considering age, diagnosis, response to medicines and previous attempts.

Is IUI painful or does it need bed rest?

You may feel brief discomfort or mild cramps during insemination. Prolonged bed rest is generally unnecessary. Follow your clinic’s instructions and report severe pain, fever or heavy bleeding promptly.
